Ozark Tri-County's AIM Statement for the Diabetes Collaborative

To continuously assess and improve Ozark Tri-County’s health care system by providing community based, high quality, cost effective care using the Care and Improvement Model which includes the community, patient and the health system and will be evidenced by the measures listed below.

• HgbA1c<7.0 (2-4 times per year)
• Documented self-management goals
• Aspirin therapy
• BP<130/80 (each visit)
• LDL ((<100)
• Dental Exam (Yearly)
• Dilated Eye Exam (Yearly)
• Comprehensive Foot Exam (Yearly)
